Person Centred Planning and Life Coaching

A totally independent service, free from any conflict of interest, Circles Network will work with individuals, their families and the services that support them (as appropriate).

For people facing transition and change we can:

  • Work closely and listen deeply to the person
  • Provide an in-depth, personal plan
  • Coach individuals and their supporters towards new horizons
  • Assist with planning for Direct Payments and individualised budgets
  • Provide advocacy and support at reviews and meetings
As an independent, third sector organisation we are well placed to work in synchronisation with other agencies, free from any conflict of interest. This has proved essential for providers of housing, residential or day care support who would not consider it ethical to take a lead role in person centred approaches.

Personalisation of care

Government priorities, outlined in the press and in recent Putting People First concordat and in the Valuing People Now report that personalisation is not an option, it will become the standing operating system. The government has given an unequivocal commitment that in future “everyone who receives social care support, regardless of their level of need, in any setting, whether from statutory services, the third and community or private sector or by funding it themselves, will have choice and control over how that support is delivered.

Our experience

Circles Network has provided assistance to thousands of disabled and disadvantaged people to develop and implement excellent person centred plans over the past 14 years. Our reputation in this field is well respected and we have also trained over 3,000 staff from other agencies in the concept of person centred approaches and life coaching.

Who might benefit?

  • Looked after children facing transition
  • Young disabled people wanting to leave home
  • People with learning difficulties receiving support
  • People with physical or sensory impairments
  • People with long term mental ill health
  • Older people facing changing circumstances

Costs

Costs will be charged at an economical rate based on the depth of the plan required, time taken and location. Please arrange a discussion with one of our representatives. Change is inevitable The first step towards a personalised approach is to have an independently produced person centred plan.
